Preparing for your first visit
You should bring the following items with you for your first visit:
Contact information from your previous dentist, so we can gather necessary records
Your insurance card
A list of all medications currently being taken, including over the counter and herbal supplements
Your completed new patient forms online or in office
We would also like to be aware of any medical conditions you have, especially heart-related conditions, as well as the medication you are taking for them. Patients under 18 should be accompanied by a parent or legal guardian.
